Complete and Fax the Federal Postcard Application to the North Carolina State Board of Elections' secure fax line 919-715-0351.  You may download this form at http://www.fvap.gov/pubs/onlinefpca.pdf.

In Section 5, where it asks where you wish your ballot to be mailed, please indicate that you want it to be faxed.  Make sure you include on the application the fax number where you wish to have your ballot faxed.  This cannot be a DSN number.

The State Board of Elections will forward your request to our office by email. 

We will process your request and fax to you:

Certificate of Absentee Voter (this must be completed)

A transmittal sheet (this must be completed) to be used when faxing to us your voted ballot and the completed Certificate of Absentee Voter

Instructions for marking your ballot and returning it to the board of elections

Your Official Ballot

Please be sure that after you have faxed your ballot and certificate of absentee voter that you immediately insert them into an envelope and mail them with proper postage to:
